- Eyelid disorder, eg, hordeolum, chalazion, blepharitis
- Inverted eyelash (eg, entropion)
- Dacryocystitis (lacrimal sac infection)
- Decreased tearing, eg, age-related, Sjögren’s syndrome, anticholinergics
- Blocked tear duct
- Sinusitis
- Migraine or cluster headache
- Temporal (giant cell) arteritis
- Ophthalmic zoster (pain may precede rash)
- Uveitis
- Scleritis or episcleritis
- Optic neuritis
