Etiology

  • Atherosclerotic heart disease (usually)
  • Coronary vasospasm
  • Hypertrophic obstructive cardiomyopathy (HOCM)
  • Aortic stenosis
  • Aortic insufficiency (severe)
  • Increased metabolic demand: hyperthyroidism, anemia, tachycardia
  • Syndrome X
  • Rare: congenital coronary anomalies, emboli, coronary artery dissection, vasculitis, e.g., Kawasaki disease or Takayasu's disease
