Idiopathic (many likely viral)
Tuberculous
Bacterial, eg, extension from pulmonary infection, Lyme

Noninfectious

Malignancy, eg, lung, breast, renal cell, lymphoma
Uremia
Dressler's syndrome (post-MI or postcardiotomy)
Radiation
Systemic lupus erythematosus (including drug-induced, eg, procainamide, hydralazine)
Rheumatoid arthritis
Scleroderma
Mixed connective tissue disease
Drugs: minoxidil, penicillins
Myxedema
