- Viral (especially coxsackie and echovirus, also influenza, Epstein-Barr, varicella, hepatitis, mumps, HIV)
- Idiopathic (many likely viral)
- Tuberculous
- Bacterial, eg, extension from pulmonary infection, Lyme
Noninfectious - Malignancy, eg, lung, breast, renal cell, lymphoma
- Uremia
- Dressler's syndrome (post-MI or postcardiotomy)
- Radiation
- Systemic lupus erythematosus (including drug-induced, eg, procainamide, hydralazine)
- Rheumatoid arthritis
- Scleroderma
- Mixed connective tissue disease
- Drugs: minoxidil, penicillins
- Myxedema
