- Atherosclerotic heart disease (usually)
- Coronary vasospasm
- Hypertrophic obstructive cardiomyopathy (HOCM)
- Aortic stenosis
- Aortic insufficiency (severe)
- Increased metabolic demand: hyperthyroidism, anemia, tachycardia
- Syndrome X
- Rare: congenital coronary anomalies, emboli, spontaneous coronary artery dissection, vasculitis, e.g., Kawasaki disease or Takayasu’s disease
