- Tissue hypoxia, eg, cardiogenic, septic, or hemorrhagic shock; seizure; carbon monoxide or cyanide poisoning
- Hepatic or renal failure
- Intestinal ischemia
- Diabetes, especially with metformin use

- Infection
- Leukemia or lymphoma
- Drugs: ethanol, methanol, salicylates, isoniazid
- AIDS
- Idiopathic
