  • Hepatic or renal failure
  • Intestinal ischemia
  • Diabetes, especially with metformin use
  • Ketoacidosis
  • Infection
  • Leukemia or lymphoma
  • Drugs: ethanol, methanol, salicylates, isoniazid
  • AIDS
  • Idiopathic
