- Malignancy, especially lymphoma, leukemia, renal cell carcinoma, primary or metastatic liver cancer
Less common causes - Cardiovascular disease, eg, myocardial infarction, thrombophlebitis, pulmonary embolism
- Gastrointestinal disease, eg, inflammatory bowel disease, alcoholic hepatitis, granulomatous hepatitis
