Etiology

Superior mediastinal tumor: lung, lymphoma, thyroid, thymoma, teratoma, synovial cell carcinoma, angiosarcoma
Constrictive pericarditis
Thrombophlebitis due to central venous catheter or pacemaker wires
Chronic fibrotic mediastinitis: idiopathic, tuberculosis, histoplasmosis, pyogenic infection, drugs
Aortic arch aneurysm
Substernal goiter
